Before an online entry can be accepted for a player under the age of 18, a signed copy of the parental consent form below must be received by the WPGA.
The following should be printed, signed by the a parent or guardian and mailed or faxed to the WPGA at:

| Eligibility |
Open to two-man teams of amateur golfers, residing in Western PA or affiliated with WPGA Member Clubs. The combined USGA Handicap Indicies of the two players must not exceed 12.0. Players do not need to be affiliated with a WPGA Member Club as long as they meet the residency requirements and have a valid USGA Handicap Index. The field shall be limited to 48 teams, and the Committee reserves the right to select the most representative field. Substitutions are permitted, provided the new entrant meets the eligibility requirements |
| Format |
36 holes of stroke play in accordance with the USGA Rules of Golf (see Rule 31 - Four-Ball Stroke play). Prizes will be awarded in two divisions, regular and Century.
Century division teams are teams where the combined age of the two players is 100 or greater. Century division teams are also eligible for the regular division. Both players ages must be specified on the entry form or in the members database in order to be eligible for the Century division.
In the event of a tie for the championship, the winners will be determined by a hole-by-hole playoff immediately after the conclusion of the final round. |
| Late Entries |
The Committee reserves the right to accept late entries. Such entries may be accepted at the convenience of the Committee, and must be accompanied by a late fee of $20 prior to acceptance. |
| List of Conforming Golf Balls |
The ball the player plays must be named on the current List of Conforming Golf Balls issued by the United States Golf Association.
PENALTY FOR BREACH OF CONDITION:
Disqualification. |
| Other Items |
Entry fees include range balls, food, prizes and cart fees. |
| Rules of Play |
USGA Rules of Golf, the WPGA Local Rules card and other Local Rules distributed will govern play. |
| Starting Times and Information |
Starting times and other information will be available on the WPGA web site (http://www.wpga.org) approximately one week before the championship. If you are unable to get your time from the website, please call 412-826-2180.
The optional condition to Rule 6-3 is in effect. See Page 27 of the Rules of Golf. |
| Transportation |
Included in the entry fees, but not required. Players who walk may request a refund of the appropriate amount from the WPGA after the event. |
| Withdrawals |
Withdrawals after the groupings and starting times have been finalized will be charged a $20 service charge. Withdrawals on the day of the event will not recieve an refund. |
