Before an online entry can be accepted for a player under the age of 18, a signed copy of the parental consent form below must be received by the WPGA.
The following should be printed, signed by the a parent or guardian and mailed or faxed to the WPGA at:

| Distance Measuring Devices |
Distance Measuring Devices which have no other functions may be used. Use of a DMD that has any function such as slope calculation will result in disqualification even if such a function is turned off. |
| Eligibility |
Open to all amateur golfers residing in Western Pennsylvania as of April 1, 2005 who are eligible for the USGA Amateur Public Links Championship (see USGA Amateur Public Links Entry Form for eligibility requirements.) Each contestant must have a USGA Handicap Index not exceeding 9.4 strokes. |
| Late Entries |
- The Committee reserves the right to accept late entries. Such entries may be accepted at the convenience of the Committee, and must be accompanied by a late fee of $50 prior to acceptance.

| List of Conforming Golf Balls |
The ball the player plays must be named on the current List of Conforming Golf Balls issued by the United States Golf Association.
PENALTY FOR BREACH OF CONDITION:
Disqualification. |
| One Ball Rule in effect |
The Optional condition requiring the use of a single brand and type of ball is in effect. See page 106 of the Rules of Golf. |
| Other Items |
Entry fees include range balls, food, prizes and cart fees. |
| Rules of Play |
USGA Rules of Golf, the WPGA Local Rules card and other Local Rules distributed will govern play. |
| Starting Times and Information |
Starting times and other information will be available on the WPGA web site (http://www.wpga.org) approximately one week before the championship. If you are unable to get your time from the website, please call 412-826-2180.
The optional condition to Rule 6-3 is in effect. See Page 27 of the Rules of Golf. |
| Transportation |
Included in the entry fees, but not required. Players who walk may request a refund of the appropriate amount from the WPGA after the event. |
| Withdrawals |
Withdrawals after the closing date will be charged a $25 service charge. Withdrawals on the day of the event will not recieve an refund. |
